Privacy Policy
How San Integrative Psychiatry PLLC protects your privacy on this website and in your care.
Effective date: October 10, 2026
Who we are and what this policy covers
San Integrative Psychiatry PLLC (“San Integrative Psychiatry,” “we,” “us,” or “our”) is an outpatient psychiatric practice in Colorado. Care is provided by Fasilat Olasumbo, MSN, PMHNP-BC, a board-certified psychiatric mental health nurse practitioner. We offer psychiatric evaluations, medication management, and supportive therapy in person and by telehealth.
This Privacy Policy explains how we collect, use, share, and protect information when you visit www.sanintegrativepsych.com, contact us, or receive care from us. It applies to:
- Website visitors: anyone who browses this site.
- Prospective patients: anyone who calls, emails, or faxes us before becoming a patient.
- Patients: people who receive care from us, and their parents, guardians, or personal representatives.
As a health care provider, we are a “covered entity” under the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is governed by HIPAA, applicable Colorado law, and our Notice of Privacy Practices, which you receive at intake and may request at any time. If this policy and the Notice of Privacy Practices ever conflict about your health information, the Notice of Privacy Practices controls.
Information collected through this website
This website is informational. It does not have patient accounts, online forms, live chat, or payment pages, and it does not ask you to enter health information.
- No tracking or advertising cookies. We do not use advertising cookies, tracking pixels, social-media pixels, session recording, or behavioral analytics. We do not sell, rent, or share visitor information for advertising.
- Server logs. Like all websites, our hosting provider automatically processes basic technical data needed to deliver pages securely, such as your IP address, browser type, the page requested, and the date and time. This data is used only to operate, secure, and troubleshoot the site, and is kept for a limited time under the host’s standard practices.
- Third-party content. Pages load fonts from Google Fonts, and “Directions” links open Google Maps. When your browser connects to these services, they may receive your IP address and browser information under their own privacy policies. Links to other sites (for example, a directory profile or a government website) are provided for convenience. We are not responsible for their privacy practices.
Visiting this website does not, by itself, create a provider–patient relationship, and we do not treat ordinary browsing data as PHI.
When you contact us
When you call, email, fax, or leave a voicemail, we collect what you choose to share, such as your name, phone number, email address, insurance information, and the reason for your call. We use it to respond to you, schedule a consultation or appointment, check insurance benefits, and coordinate your care.
Please do not send detailed health information by regular email or voicemail. Standard email is not encrypted end to end and could be read by others. For general questions, email info@sanintegrativepsych.com. For anything involving your health, call (303) 304-5795. Once you are a patient, we will discuss secure ways to communicate. If you ask us to communicate by unencrypted email or text, we may do so after explaining the risks, as HIPAA allows.
Other health care providers may send records and referrals by fax to (888) 649-7004.
Information we collect from patients
To provide safe, effective psychiatric care, we collect and maintain information such as:
- Identifying and contact information, date of birth, and emergency contacts
- Insurance and billing information
- Mental health and medical history, symptoms, diagnoses, screening results, and treatment plans
- Medications, prescriptions, allergies, and lab results
- Records received from other providers, with your authorization or as the law permits
- Prescription monitoring program (PDMP) information, when required or appropriate before prescribing certain medications
- Notes from your visits, whether in person or by telehealth
Patient records are kept in a secure electronic health record and practice-management system operated by vendors that sign HIPAA Business Associate Agreements with us.
How we use and share health information
We use and disclose PHI only as HIPAA and Colorado law permit. In general, we may use or share your information without your written authorization to:
- Treat you: for example, consulting with your primary care provider or therapist, sending prescriptions to your pharmacy, or ordering labs.
- Get paid: for example, submitting claims to your health plan or confirming eligibility and benefits.
- Run our practice: for example, quality review, scheduling, appointment reminders, and working with Business Associates that support our operations under written confidentiality agreements.
- Meet legal and safety duties: for example, reporting suspected child or elder abuse or neglect, responding to a valid court order, cooperating with health oversight agencies, or acting to prevent a serious and imminent threat to the health or safety of you or others.
We will ask for your written authorization before using or sharing your information for other purposes. This includes any marketing, any sale of PHI, and most disclosures of psychotherapy notes. You may revoke an authorization in writing at any time, except for actions already taken in reliance on it.
We never sell your personal or health information.
Your privacy rights
As a patient, you have the right to:
- See and get a copy of your health records, including electronic copies, generally within 30 days of your request. We may charge a reasonable, cost-based fee.
- Ask us to correct information you believe is incorrect or incomplete.
- Request confidential communications, for example asking us to call a specific number or mail to a different address.
- Ask us to limit what we use or share. We are not required to agree, except that we must agree if you pay for a service in full out of pocket and ask us not to share it with your health plan.
- Get a list (an “accounting”) of certain disclosures we have made.
- Get a paper copy of our Notice of Privacy Practices.
- Choose someone to act for you, such as a legal guardian or a person with your medical power of attorney.
- Be notified if a breach compromises your unsecured health information.
To make a request, contact us using the information below. We may need to verify your identity first.
Children and families
We see children and teens with the involvement of a parent or legal guardian. In most cases, the parent or guardian acts as the child’s personal representative and may access the child’s records, subject to Colorado law and the child’s safety and best interests. When couples or family members take part in a patient’s care, we share only what is appropriate for that care or what the patient has authorized.
This website is not directed to children under 13, and we do not knowingly collect personal information online from children.
Cookie policy
This website does not set cookies of its own and does not use advertising, analytics, or tracking cookies. Third-party services the site loads, such as Google Fonts, or opens, such as Google Maps, may use cookies or similar technologies under their own policies when you interact with them. You can block or delete cookies in your browser settings without affecting how this site works. If we ever add analytics or other cookies, we will update this policy first.
Telehealth
Telehealth visits are held on a secure, HIPAA-compliant video platform. For your privacy, please join from a private location, use a personal device and a secure internet connection, and avoid public Wi-Fi. Telehealth is available only to patients who are physically located in Colorado at the time of the visit.
How we protect information
We use administrative, physical, and technical safeguards designed to protect your information, including access limited to authorized staff, password-protected and encrypted systems, secure storage of any paper records, and Business Associate Agreements with vendors that handle PHI. No method of transmission or storage is completely secure, but we work to protect your information and will notify you and the authorities as required by law if a breach occurs.
How long we keep records
We keep health records for as long as Colorado and federal law require, and longer if needed for your continuing care or a legal matter. Records of minor patients are kept at least until the required period after the patient reaches adulthood. When records are no longer needed, they are destroyed securely.
Questions and complaints
If you have questions about this policy or believe your privacy rights have been violated, please contact our Privacy Officer, Fasilat Olasumbo, MSN, PMHNP-BC. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
